Documentation of completed immunizations should be submitted via the Patient Portal.
If you are unable to access the online forms, you may also print the Health History Form or International Health History Form and either have this completed by a provider or attach your official documentation. This information can be submitted via fax or mail.

If you have not been immunized but are immune based on blood tests or having had the diseases, you must submit official documentation from your medical provider (personal physician, pediatrician or county health department) that you had the respective disease(s) or blood test results.
On May 4, 2021, OSU announced its intent to require COVID-19 vaccinations for students and employees who learn, work or engage with others in-person during classes or university activities. Students can now comply with this policy by confirming their vaccination status through the patient portal. Information on seeking exemptions to this requirement can be found by scrolling down this page or clicking "student" below.
Students must comply with this requirement by Sept. 15, 2021.

STUDENTS WILL NEED to complete ONE of the following steps to meet this requirement:
Confirm that you have been vaccinated by uploading your card to the patient portal. A vaccine received in another country will be acceptable as long as the vaccine received is approved by the country in which it was administered. Or...
Confirm by completing a form if you have lost your card and upload it to the patient portal. Or...
Claim a medical or religious/philosophical belief exemption from the vaccination requirement by completing the appropriate form declining to comply.
