Door Access Using ID Cards

To request access to specific offices or areas on campus not already assigned to your ID card please follow the instructions on the Facility Access Request page.

Replacement Fee

The first ID card is free of charge. The fee for a replacement card is $25. Requests for a replacement ID card can be made at the Mail & ID Center. ID cards that are determined to have malfunctioned due to a manufacturing flaw or age will be replaced free of charge. ID cards should last for approximately four years without the need for replacement.

Please do not punch holes in your ID card or otherwise damage the magnetic strip or barcode.  Punching holes in the card will likely cause damage to the electronic circuitry contained within.

Lost and Found ID Cards

Lost or stolen ID cards should be immediately reported to the OSU-Cascades Department of Public Safety to protect your card account from unauthorized use. Any use of funds before an ID card is reported lost/stolen will be the responsibility of the cardholder.

If you find an ID card that is not yours, please turn it in to the Department of Public Safety.
